Forms
No department would be complete without numerous forms to fill out. Samples of the forms you will encounter and need to complete during your graduate studies are presented after this message. Copies of these forms are available from the department's Graduate Secretary by calling 508-831-5231 or as Web Documents linked to this page.
Program of Study
Two forms are included here. One is for the M.S. degree (PDF) and one is for those in the Ph.D. program (PDF) .
Make sure that you have your advisor sign the form before you give it to the department Graduate Coordinator.
Graduate Petition Form
The WPI Graduate Petition Form ( PDF) is used to ask for variances to universityand department rules and restrictions, particularly those that require changes of any sort to your transcript. Fill out the form completely and add all necessary supporting documentation. Have your academic advisor sign the form and turn it into the department Graduate Coordinator. Examples of the use of this form include the acceptance of transfer credit and posting of that credit to a student's transcript, and requesting the waiving of a campus rule (e.g. the eight-year completion rule) the acceptance of different Courses as alternatives to admission restrictions (for those provisionally admitted), changing directed Research credit to M.S. thesis credit (and the reverse), and pre-approval of Courses to be taken at another University.
It is strongly suggested that any student who plans to file a petition form with the department, first check with the Graduate Coordinator to discuss the reason for the petition and how to minimize problems that might occur.
Application for Graduation
The official Application for Graduation (PDF) includes all pertinent information required for either the M.S. or Ph.D. degree. This form is particularly important for the Ph.D. students since it shows the dates of the various Ph.D. exams. Please make sure that the form is completely filled out and approved by your academic advisor before you turn them in to the Department Graduate Coordinator. Two of the most common errors in filling out the Application for Graduation form is listing Courses that were not on the student's Program of Study, and listing Courses that were dropped (usually in the final term). Contact the ECE graduate secretary for a copy.
Thesis/Dissertation Approval Form
Students completing a Master's Thesis and students finishing their Doctoral Dissertation must submit their final theses/dissertations electronically and turn in a signed copy of the title page, along with the ETD Approval Form, to the ETD Coordinator.
